Waiver & Consent

Participation & Medical Treatment Waiver

I/We, the parent(s)/guardian(s) of the above-named camper, hereby grant permission for the camper to participate in all activities offered at Elbethel Bible Camp Youth Camp 2026, including but not limited to swimming, hiking, sports, and team activities.

In the event of illness or injury, I authorize Elbethel Bible Camp staff to obtain necessary emergency medical treatment for my child. I agree to be responsible for any costs incurred as a result of such treatment.

I acknowledge that participation in camp activities carries inherent risks and I voluntarily release Elbethel Bible Camp, its staff, volunteers, and directors from any and all claims, demands, and liabilities arising out of participation in the program, except in cases of gross negligence.

Photo/Video Consent: I consent to photographs and video recordings of my child being taken during camp activities and used for Elbethel Bible Camp promotional materials, social media, and website.

I confirm that all information provided in this registration form is accurate and complete to the best of my knowledge.

I agree to the Participation & Medical Treatment Waiver and grant permission for my child/the camper to attend Elbethel Youth Camp 2026. *
